Acorns Forms for Parents

Please see below forms which you may need for your child when they are in Acorns Class.

Medication Forms

Please see below forms which you may require should you wish the school to administer medication to your child.
School is able to administer prescription medication which must be in the original container.
School is occasionally able to administer non-prescription medication, this should be discussed with the headteacher or deputy headteacher before being given to school.

If your child has a long term medical condition or illness then please arrange a meeting with the Headteacher via the school office.  It would be useful if you could complete the individual healthcare plan before attending the meeting.

If your child uses an inhaler for asthma, we hold an emergency inhaler.  You will need to complete the form below in order for us to be able to administer the emergency inhaler to your child.
